Fees and Payment

What is the cost of an individual counseling session?

I agree to provide individual counseling for the fee of $115 per 50 minute session.

The fee for each session will be due at the conclusion of the session.  Cash, credit cards and personal checks are acceptable for payment, and I will provide you with a receipt of payment.  The receipt of payment may also be used as a statement for insurance if applicable to you.  Please note that there is a $30 fee for any returned checks.

What is the cost of a group counseling session?

I agree to provide group counseling at the rate of $45 per 90 minute group therapy session.

What is the cost of individual supervision for Associate Professional Counselors (APC)?

My fee for individual supervision is $90 for 60 minutes, on a weekly basis, and $130 for 90 minutes on a biweekly basis.

Do you accept insurance?

Yes. Currently, I am in network with Aetna and Anthem/ Blue Cross Blue Shield insurance companies.

You should carefully read the section in your insurance coverage booklet that describes mental health services. If you have questions about the coverage, call your plan administrator. You (not your insurance company) are responsible for full payment of my fees.

You should also be aware that most insurance companies require me to provide them with a clinical diagnosis in order to provide reimbursement for counseling services rendered.  This will be provided to them after each session. I am more than happy to discuss any diagnosis with a client if so desired.

Once we have all of the information about your insurance coverage, we will discuss what we can expect to accomplish with the benefits that are available and what will happen if they run out before you feel ready to end our sessions. It is important to remember that you always have the right to pay for my services yourself to avoid the problem described above. You are also financially responsible for any changes to your insurance coverage and notifying me before a session if there are any changes to your insurance plan. Any balances that are not paid by the insurance company will be rendered to you via an invoice for you to pay.

Please note that insurance companies do not reimburse for missed sessions.

Some insurance companies may reimburse individuals for appointments with out-of-network providers. If I am not in network with your insurance company, I will be glad to provide you with a statement for your insurance company and to assist you with any questions you may have in this area. Insurance companies have many rules and requirements specific to certain plans. Unless otherwise negotiated, it is your responsibility to find out your insurance company’s policies and to file for insurance reimbursement.

Do you have a cancellation policy?

Yes.  In the event that a client is unable to keep an appointment, a client must notify me at least 24 hours in advance.  If such advance notice is not received, a client will be financially responsible for the session they missed.  Please note that insurance companies do not reimburse for missed sessions.
